Before opening the form
A release of information records who you want the practice to communicate with, what may be shared, and the purpose. Review those choices carefully before signing. Signing does not automatically give another clinician access to your full chart or patient portal.
Have the other person’s or practice’s name and contact information ready. If anything is unclear, call (509) 356-2424 before submitting.
The form is provided by HIPAAtizer. It loads only when you choose to open it. This page does not use Google Analytics.

Contact the practice through the patient portal or by phone. The Notice of Privacy Practices explains authorization and revocation.
