Meet Udoka Addy.
MSN, ARNP, PMHNP-BC · Board-certified psychiatric nurse practitioner · Air Force veteran
I provide psychiatric evaluation and medication management, with time to understand your life as well as your symptoms. We discuss what you’ve tried, what matters to you, and what might help. You work with me at your first evaluation and follow-ups.
Free consultation by video or phoneWho I work with
Adults 18 and older seeking help with depression, anxiety, trauma and PTSD, bipolar disorder, grief, sleep concerns, or stress. Veterans are welcome. You must be physically in Washington during appointments. If you already have a therapist, we can coordinate with your permission.
Care can include symptom measures, nonjudgmental substance-use screening, tobacco-cessation support, and coordination with your therapist or primary care clinician when relevant and authorized. The plan follows your needs.
Appointments & payment
Start with a free 15-minute video or phone consultation. Initial evaluations are 60 minutes by video; standard follow-ups are 30 minutes.
Insurance participation is not yet active. If you hope to use insurance for future care, you can join the insurance interest list. For appointments before participation is active, see current fees and payment details.
Before booking a paid appointment, review the current fees and cancellation policy. Visit type and cost are agreed in advance.
Before your video visit
- Complete the requested intake forms in SimplePractice.
- Have a current medication list, including doses and supplements, and your pharmacy information ready.
- Choose a private place in Washington where you can speak without being overheard. Do not join while driving.
- Check your internet, camera, and microphone. Use the unique SimplePractice link for this appointment, or join from the appointment in your patient portal. Do not reuse a previous appointment’s link.
If your connection fails, call (509) 356-2424.
On a computer, you can join in your browser. On a phone or tablet, follow the link’s app instructions. See SimplePractice’s computer guide or phone and tablet guide.
For a phone appointment
Choose phone or video for the free consultation. For a phone appointment, I will call the phone number you provide at the scheduled time. Check that your number is correct in the patient portal and be ready in a private place in Washington. No video link is needed. If we cannot connect, call (509) 356-2424.
Phone follow-ups can be arranged when we agree they are clinically appropriate and legally permitted. Contact me through the patient portal before choosing this format. We will agree on the visit type and fee in advance; a video visit, or an in-person assessment with another provider, may be needed when a phone visit cannot provide an adequate assessment.
Recording & AI note-taking
Audio recording or AI transcription is optional and requires separate consent before use. Agreeing to telehealth is not permission to record. You can decline or ask to stop; I will document manually, and your access to care will not depend on agreeing.
Between appointments
Use the SimplePractice patient portal for non-urgent clinical questions. Secure portal messaging is included. I respond within 24 hours, including weekends. Contact the practice before running out of medication so there is time to review the request; refills are not guaranteed without review.
Records & coordination
Complete a release of information when you want this practice to coordinate with another person or clinician. Read the Notice of Privacy Practices for your rights and how your records may be used.
For the complete payment details, including reimbursement questions and your right to a Good Faith Estimate, see fees and insurance.
Medication decisions
Controlled medications are within my scope when clinically indicated. I do not start them at a first visit or prescribe them on request. Continuing an existing prescription requires a fresh diagnostic review, a Prescription Monitoring Program review, a treatment agreement, and regular follow-up.
