Send a short request
In the secure form, use “What would you like to ask or let me know?” to include:
- Your insurer and plan name, if you know it.
- Whether you prefer a phone call or email about availability.
- Whether you can wait or are looking for care sooner.
- Your permission for me to contact you about the interest list.
You do not need to include your member ID, an insurance card, or a clinical history for this request.
Wording you can use
Please add me to the insurance interest list. My insurer and plan: ___. Preferred contact method: ___. I can wait / I’m looking for care sooner. You may contact me about this request and future availability with my plan.
Complete the contact details and required questions, then send your message. I reply within 24 hours, including weekends. My reply will confirm your request; opening the form does not add you to the list.
